The Pharmacovigilance Committee supports the identification, documentation and reporting of suspected adverse drug reactions arising during patient care.
The Committee promotes awareness among healthcare professionals regarding medicine safety and appropriate ADR reporting practices under the applicable pharmacovigilance framework.
Adverse Drug Reaction Reporting
Suspected adverse drug reactions should be identified, documented and reported through the appropriate institutional and pharmacovigilance channels.
Identify
Recognise and assess a suspected adverse reaction during patient care.
Document
Record relevant clinical and medicine-related information required for evaluation.
Report
Submit the suspected ADR through the designated reporting mechanism.
